How do I make a custom mailing list application for my Myspace page?

I’d like to make a mailing list application on my myspace musicians page (http://www.sevenyearnight.com). I don’t want to have to sign up and pay any fee, I just want the html code for a mailing list application/box.

    I suggest making the form hosted on a separate domain, and then linking to it on your myspace.
